The registration form is to register you for the weekend's events. It is not for hotel registration – you will need to reserve your hotel room by contacting the hotel directly (please visit our Hotel Information page.) Review the instructions on the forms carefully. If you are a 'First Year Survivor' attendee, please see the information below.
Event check-in is from 3 p.m. to 7 p.m. on Friday, September 30, 2016 at the Montego Bay Resort & Conference Center. Welcome bags will be available during that time. If you plan to arrive after 7 p.m., please let us know so we can have your registration set aside. If you are arriving on Saturday, please pick up your welcome packet on the Montego Bay 3rd floor oceanside pool sun deck. Extended: Please register by September 12, 2016.

If your fallen officer or firefighter has not yet been recognized at the MAPFSW event, you are considered a first-year attendee. If you would like your hero to be recognized at the vigil on October 1, please include the 'First Year Attendee Submission' form (below). The hero's death must be considered 'line-of-duty' and you must be a recognized family member to submit the form – we want to make sure that we honor your hero appropriately. This form must be submitted along with your registration form.
If you are submitting the ‘First Year Attendee Submission’ form, up to two rooms at the Montego Bay will be paid by the MAPFSW committee. Please contact the North Wildwood Police Dept. at 609‐522‐2030, ext. 1504 and let them know you are a first‐year MAPFSW attendee and need to make hotel arrangements. This only applies to first‐year honorees. Extended: Please register by September 12, 2016.
